Breast Surgery

Breast enhancement is one of the most common aesthetic plastic surgery procedures all over the world.

Breasts can be enlarged for aesthetic reasons either by using silicone implants, or by filling the breast with the fat which is commonly harvested from tummy (abdomen); flanks; thighs; and buttocks. Silicone implant based breast enlargement is the most common method.

Breast implants

There are many different techniques of inserting breast implants. It is mainly determined by the plastic surgeon in consultation with the patients preferences. Below is a summary of the different techniques we use:

1. Infra-mammary Approach (underneath breast crease approach)

A small incision is made within the crease where the front of the chest wall comes together with the lower pole of the breast. With this approach, any type of implant can be inserted either in front or behind the chest wall (pectoral) muscle. The incision is approximately 4cm in length and Mr Vadodaria uses absorbable skin sutures.

The scar following this type of incision is hidden underneath the breast and is hardly visible in standing or sitting positions. This is the commonest approach used by most cosmetic surgeons, and this is usually our recommended approach for aesthetic breast enlargement procedures.

2. Peri-areolar Approach (through the nipple areola complex)

Areola is an area which surrounds the projection of the nipple. It is slightly darker or pinker in comparison to the colour of the breast skin.

If the surface are of areola is large enough, then implant can be inserted from this approach. One advantage of this approach is that it can also be combined with breast lift at the same time.

3. Axillary Approach (through the armpit)

Some women and some surgeons prefer using axillary approach for insertion of breast implant. The incision is made within the hairy part of the armpit and the implant is normally positioned underneath the chest wall (pectoral) muscle, however the implant can also be positioned in front of the chest wall muscle.

This is not a very common approach, with a disadvantage that the scar can be visible since it cannot be covered by a bikini while sunbathing.

Placement of Silicone Implant

Once the incision is made into the skin, the surgeon will create a pocket for the insertion of an appropriate size and type of silicone implant. The positioning of the implant is determined by the breast volume; patients’ occupation; history of previous breast enlargement surgery; and particularly preference of the surgeons and patient.

There are four types of pockets in which the silicone implant can be positioned for aesthetic breast enlargement:

Sub-glanular Pocket (behind the breast in front of muscle)

This is one of the most common type of pockets used for breast enlargement surgery. The surgery is relatively simpler and the post-operative pain following the procedure is less that when the implant is inserted behind the muscle. We use this approach when there is sufficient breast tissue to cover the silicone implant.

Sub pectoral/sub muscular pocket (behind the chest wall muscle)

There are many variations in creating the pocket underneath the pectoral muscle based on the extent of the release of chest wall muscle from the inner aspect of the lower ribs. Mr Vadodaria uses this approach when his patient does not have sufficient breast tissue to cover the implant.

Dual Plain

This is a variation of sub-glanular and sub muscular pocket creation. This is very commonly used by Mr Vadodaria in his practice. A sub-muscular pocket is created, the pectoral muscle attachment is released from the inner aspect of the lower ribs, and a limited pocket is created underneath the breast tissue but in front of the pectoral muscle. This type of modification allows the implant to rest into the lower pole of the breast. This is a useful pocket in which the upper pole of the silicone implant is well covered by the muscle hence the edge of the silicone implant is less likely to be felt by the patient especially in the upper part of the breast.

Sub-fascial Pocket

The front of the chest wall (pectoral) muscle is covered by a thin sheet of shiny white tissue called pectoral fascia. A pocket is created underneath the breast tissue between the pectoral fascia and the muscle so that implant is additionally covered by the fascia underneath the breast tissue.

Shape of Breast implants

The silicone implants are available in different sizes; different shapes; and different types of projection. Implants are also available with different consistency.

The selection of breast implant depends upon the size; shape; and the level of the breast. The final decision is dependent on clinical need and the preferences of the patient.

Round Implants

Round implants are the commonest implants used for breast enlargement. They are cost effective, and they provide satisfactory outcomes if the selection as well the procedure are performed appropriately. The operation is relatively simpler using round silicone implants and will not change the shape of the breast if the implant rotates.

Anatomical Shape (teardrop shape) Breast Implant

There are certain uses of anatomical implants, particularly in the correction of tubular breasts with constricted lower pole. Anatomical implants are expensive and the use of anatomical implants increases the chances of revisional surgery since there is distortion of the shape of the breast if implant rotates within the breast.

Profiles (projection)

Implants are available with different degrees of projection. The projection of the implant is determined by various proportion of the circumference of the base of the implant and the height of the implant. Implants are available with low; medium; high; and ultra-high projection. In general terms, high profile implants are used if a woman has got relatively narrow chest wall, but would like to have relatively bigger breast enlargement. Size, shape, and type of the projection, will be determined and discussed when you come for an initial consultation with us.

Small circumference and bigger height will give more projection and big circumference and smaller height will give low projection.

The Stability of Implants

The stability of the breast implant is dependent on the viscosity of silicone gel used within the breast implant. Saline filled breast implants, such as the controversial PIP implants are the least stable implants. If the implant is stable then the chances of kinking is minimized. We only use the highest quality implants, and do not use saline filled implants in our practice for aesthetic breast enlargement.

Influence of Selection of Breast Implant in Asymmetrical Breast

Scientific studies have shown that 70% of women are likely to have some form of breast asymmetry either in the form of size, shape, or lavel. Many times there may be a combination of breast asymmetry in terms of size, shape and/or level. In such cases different volumes, projections, and shape of the implant is selected to improve breast asymmetry.

Surface of Silicone Implants - The silicone implants are available with smooth surface or textured rough surface. Textured surface is believed to reduce the incidence of capsular contracture and hence less chances of hardening of the breast. We use textured rough surface breast implants to minimise incidence of capsular contracture.
